Simply download the health questionnaires package below, fill the appropriate form out from your client's medical history (all of the relevant questions are provided) and fax it back to our Offices in San Diego. Once received ... we will then review it for completeness and securely forward it directly to AMERICO for underwriting. Their common sense approach to underwriting studies each applicant individually for the best possible coverage. After a prelim has been reviewed by AMERICO ... we will fax their response directly back to you.
